cuz 6d498d8187 Use external symbols for the CBM kernal jump table functions. This allows 
to emulate these functions on platforms where one or more of these functions
are not available (PET, CBM-II).

Implementation notes:

CONDES routines with a type code of 2 are used to implement interrupt
handlers. Note that these are called as soon as the program is started,
so they have to cope with this.
