IR: Split out DebugInfoMetadata.h, NFC

Move debug-info-centred `Metadata` subclasses into their own
header/source file.  A couple of private template functions are needed
from both `Metadata.cpp` and `DebugInfoMetadata.cpp`, so I've moved them
to `lib/IR/MetadataImpl.h`.
