Reverted in r231254 due to a self-hosting crash of Clang (see Clang
PR22793). Workaround the crash by using {} instead of = default to
define a dtor.

Since OptionValue (& its base classes) have user-declared dtors, use of
the implicit copy ctor/assignment operator is deprecated in C++11.
Provide them explicitly (defaulted) to avoid depending on this
deprecated feature.

These objects are never polymorphically owned, so there's no need for
virtual dtors - just make the dtor protected in the base classes, and
make the derived classes final.

There doesn't seem to be any need to assert that iterator assignment is
between iterators over the same node - if you want to reuse an iterator
variable to iterate another node, that's perfectly acceptable. Just
don't mix comparisons between iterators into disjoint sequences, as
usual.

There are static variables of this around that we really want to go
into a read-only segment. Sadly compilers are not smart enough to figure
that out without constexpr.

Summary:
These ISA's didn't add any instructions so they are almost identical to
Mips32r2 and Mips64r2. Even the ELF e_flags are the same, However the ISA
revision in .MIPS.abiflags is 3 or 5 respectively instead of 2.

Introduces a subset of C++14 integer sequences in STLExtras. This is
just enough to support unpacking a std::tuple into the arguments of
snprintf, we can add more of it when it's actually needed.
Also removes an ancient macro hack that leaks a macro into the global
namespace. Clean up users that made use of the convenient hack.

Otherwise we will always select the generic version for e.g. unsigned
long if uint64_t is typedef'd to 'unsigned long long'. Also remove
enable_if hacks in favor of static_assert.

On Windows, we now use RaiseException to generate the kind of trap we require (one which calls our vectored exception handler), and fall back to using a volatile write to simulate a trap elsewhere.

5 minutes is an eternity, so try to strike a better balance between
waiting long enough for any reasonable module build and not so long that
users kill the process because they think it's hanging.
Also give the client a way to delete the lock file after a timeout.

Use definition file for `DW_VIRTUALITY_*`. Add a `DW_VIRTUALITY_max`
both for ease of testing and for future use by the `LLParser`.
