Eli Friedman fc21f8ff14 Canonicalize insert/extractelement from single-element vectors into 
bitcasts.

It would also be possible to canonicalize the other way; does anyone 
have a preference?

dated tests are actual bug tests, whereas the named tests are used to test
for features that the this pass should be capable of performing.
