a6594fc715 mcstreamerize .file and .file. This also fixes an issue where the 
normal form of .file would fail if the filename had a weird character
in it.

4cf202ba06 remove one form of EmitString, just use EmitBytes instead. We must 
be careful to add a \0 at the end though, because EmitString didn't
do this.

42aafd7e57 If a instruction belongs to another function (and not current function) as per debug info attached with the instruction then ignore the dangling lexical scope of this instruction. Such scopes are unreachable.

c366f83d89 Refactor code that finds context for a given die. 
Create global variable DIEs after creating subprogram DIEs. This allows function level static variable's to find their context at the time of DIE creation.

1d5cc1db59 Emit method definition DIE at module level (even for methods with inlined functino body at soure level) so that the debugger can invoke it. This fixes many test failures in gdb test suite.

a2f20b20a8 Add getFrameIndexReference() to TargetRegisterInfo, which allows targets to 
tell debug info which base register to use to reference a frame index on a
per-index basis. This is useful, for example, in the presence of dynamic
stack realignment when local variables are indexed via the stack pointer and
stack-based arguments via the frame pointer.

6f01d9c2f0 There is no need to use FoldingSet to unique DIEs. 
DIEs are created from MDNode, which are already uniqued. And DwarfDebug already uses ValueMaps to find and use existing DIE for a given MDNode.

bdf45cbe15 Do not held on to DenseMap slot accross map insertion. The insertion may cause the map to grow rending the slot invalid. 
Use this opportunity to use ValueMap instead of DenseMap.

ac1ceb3dd3 Extract scope information from the variable itself, instead of relying on alloca or llvm.dbg.declare location. 
While recording beginning of a function, use scope info from the first location entry instead of just relying on first location entry itself.

c525472828 Check invalid debug info for enums. This may happen when underlyng enum is optimized away. Eventually DwarfChecker will clean this up during llvm verification stage.
