Dan Gohman d5826a33a5 Use the new tool_output_file in several tools. This fixes a variety
of problems with output files being left behind or output streams
being left unclosed. Fix llvm-mc to respect the -o option in all
modes, rather than hardcoding outs() in some cases.

// This utility may be invoked in the following manner:
// llvm-dis [options] - Read LLVM bitcode from stdin, write asm to stdout
// llvm-dis [options] x.bc - Read LLVM bitcode from the x.bc file, write asm
// to the x.ll file.
// Options:
// --help - Output information about command line switches
