Cleaned up dependencies on controller manager (#964)
* Cleaned up dependencies on controller manager

* Removed global fields

* Simplified setting up RascsiResponse and RascsiExecutor

* Got rid of remaining raw pointers

* Use references instead of pointers

* Improved encapsulation
