Kelvin Sherlock a4c2d362c5 --trace-macsbug improvements ...

1. actual macsbug names weren't supported.
2. --trace-macsbug was also printing toolcalls
3. unify the macsbug name fetching code since it was implemented
multiple times
4. add safe read functions so disasm won't trigger memory breakspoints.
